Adoption Home Studies Information STATEMENT OF SERVICES OBJECTIVE: To assess the prospective adoptive family’s ability to raise a child. THE DKM EVALUATORS: The evaluator will gather information through family meetings and other methods to assess the prospective family’s ability to raise a child. Once all the information has been compiled, a report will be written to summarize the information obtained, to verify that the applicant's meet all of the requirements for adoption, and to make a recommendation in regards to the applicant's ability to raise a child. The evaluator does NOT place children into care nor makes the final decision in regards to the placement of a child in the prospective adoptive family’s care; this decision lies within the adoption agency. The evaluator’s role is NOT to provide therapy. BEGINNING YOUR DKM ADOPTION HOME STUDY: Please contact a DKM Home Studies Evaluator to initiate the evaluation process. An application form will be mailed to you; it will include general demographic information (Name, DOB, race, employment, etc) and forms for you to complete to screen for reports of abuse or neglect through the Department of Social Services, a criminal background check through the Division of Criminal Investigation, and a health form from your physician. After the information has been returned to the evaluator, the evaluator will contact you to set up meetings in your home. During the first meeting, the evaluator will further discuss the evaluation process. DKM ADOPTION HOMES TUDIES INCLUDE: •Home visits •Interviews with the prospective adoptive family including children and other household members •Observations of the prospective adoptive family’s relationship with their own children •Review of legal documents •Review of health reports •Review of criminal record from the Division of Criminal Investigation •Review of abuse and neglect screening from the Department of Social Services •Review of collateral information collected •Review of any other information central to custody evaluation CONFIDENTIALITY: The usual rules of confidentiality do not apply in an adoption home study.
